Services Disclosure and Agreement

This document contains important information about Radiant Life Consulting LLC. Please read it carefully. Any questions will be addressed prior to starting services. Using our services represents an agreement between us.

Services Provided

Kristy Parke is an intuitive coach and energy medicine practitioner, certified in Eden Energy Medicine, Usui Holy Fire III Reiki (Master level), and the Emotion Code. She combines practical coaching with energy work to help clients navigate major life transitions and leadership challenges.

Important Limitations

Energy medicine and intuitive coaching are NOT substitutes for medical or mental health treatment by a licensed professional. We do not diagnose or treat medical or mental health disorders. If you have a diagnosed condition or symptoms requiring evaluation, our services should be used only in conjunction with appropriate licensed care.

While our methods are gentle and noninvasive, physical or emotional aftereffects may occasionally occur (dizziness, nausea, anxiety). If any procedure causes discomfort, tell us immediately and we will stop.

Professional Fees

For Individuals:

• Clarity Session (90 minutes): $300

• Transition Support (3 months, 6 sessions): $1,400

• Deep Transformation (6 months, 12 sessions): $2,700

For Organizations:

• Executive Coaching: $1,200/month per person (2 sessions)

• Team Leadership Program: Custom pricing

• Corporate Training & Workshops: Contact for pricing

Payment & Cancellation

• Executive coaching: Monthly fees due on the 1st of each month

• Payment methods: Credit card, Venmo (@kristyparke), PayPal (paypal.me/radiantlifeenergy)

• Cancellation: 24-hour advance notice required or you will be charged for the session

• Unpaid accounts over 60 days may be sent to collections

Confidentiality

Your sessions are confidential. We cannot share information about you without your written permission, except in these situations required by law:

1. Suspected abuse of a child, elderly person, or disabled person

2. Threat of serious harm to another person

3. Risk of self-harm

4. Sexual misconduct or impairment by another healthcare provider

5. Court order in legal proceedings

6. When working with multiple family members (limits discussed in advance)

7. Professional consultation (your identity is protected)

Records & Communication

• We keep brief session records, maintained securely for 7 years

• Email is not completely confidential; we use secure methods when possible

• For emergencies, call 911 or go to the nearest ER

Text Messaging

You may receive appointment reminders and follow-ups via text message. Reply STOP to unsubscribe. Message and data rates may apply. Your information will never be shared or sold.

Touch in Sessions

Some energy work involves touch. You remain fully clothed, and we explain beforehand where we will touch. You can decline at any time. Touching in a sexual manner is illegal and will never occur.

If you have a history of trauma, abuse, dissociation, or certain mental health diagnoses, please inform us before we begin so we can discuss any concerns about touch.

Terminating Services

You may end services at any time. We may terminate services if: (1) we cannot provide appropriate care, (2) you threaten or harass us or our family, or (3) you attend a session under the influence of substances.

Your Rights

• Ask questions about anything in our work together

• Request a different practitioner or referral

• Discuss concerns or complaints directly with us

Legal Proceedings

If you are involved in legal proceedings that require our participation, you will pay $350/hour for preparation and testimony. Note: healing work may affect the impact of trauma-related testimony.

Minors

If under 18, parents/guardians may have the legal right to access records. We request that they waive this right and will provide only general information unless there is a serious risk of harm.

Reminder

Energy medicine and intuitive coaching support your body's natural healing mechanisms. They do not diagnose, treat, or cure illness.
